Community Living Services
Pathway Services Unlimited provides residential services to individuals
with developmental disabilities and other physical and mental challenges
in a variety of home settings. Some individuals receive services in their
own homes or apartments, in homes shared with others or in the natural family home.
Supports and services for individuals who reside in these homes are designed
and tailored to the specific needs and desires of the individual. The Residential
Program is a supportive service designed to contribute to a successful, vital,
and meaningful life by:
- Enhancing the skills and abilities of each resident.
- Assisting individual residents in becoming a part of their community in ways that are meaningful to them.
- Providing opportunities that add to the depth and fullness of life.
- Appreciating creativity.
- Attaining personal goals.
Program Specific Admission Criteria
- Minimum age of 18 years.
- Need for active treatment; this is defined as (a) the acquisition of the behaviors necessary for the individual to function with as much self determination and independence as possible; and (b) the prevention or deceleration of regression or loss of current optimal functional status.
- Informed consent from the individual or guardian to participate in the program.
- Agreement from the individual or guardian to participate in developing and implementing a plan for services for the individual.
- Absence of behaviors which pose a danger to self or others and ability to benefit from supports and services.
Services Offered
Services offered to individuals who reside in Pathway Services Unlimited
homes are determined by their interdisciplinary team. The team is
composed of the individual, his/her guardian, key staff from agencies
who provide medical, social, habilitative services and individuals of
the person's choice.
Typical services offered include assistance as needed in the following areas:
- money management
- cooking skills
- laundry skills
- medication skills
- appropriate social skills
- assistance in daily living skills
- choice/decision making skills
Services are designed to reflect the needs and desires of the persons receiving services and provide individuals with their own personal space, privacy and security.
