Home-Based Support Services

Through the HBS Program individuals and their families or legal guardians will experience decision making in order to enhance personal growth.

Risk vs. Choice is addressed in the Individual Service Plan.

Program Specific Admission Criteria
  • Resides in Morgan, Scott, Cass, Calhoun, Greene, or Jersey county
  • Be between the ages of 18 and 60
  • Need home-based support services, but not require 24 hour a day supervision
  • Be eligible and must have applied for federal Supplemental Security Income (SSI) or federal Social Security Disability Income (SSDI)
  • Not receiving CILA services.
Staff Qualifications
The Service Facilitator is a Qualified Mental Retardation Professional (QMRP) with at least a Bachelor’s Degree in a Human Service field and has a minimum of one year’s experience providing services for individuals with developmental disabilities. The Personal Support Worker (PSW) is someone the individual or family select to assist you with daily activities. A PSW can come to your home for a few hours each week or several hours daily. If hired by the agency, PSWs must be at least 18 years of age and have a High School Diploma or equivalent and a valid driver’s license with proof of insurance.
